I HERBERT HENRY DOUGLAS-WITHERS,heretofore called and known by the name of
Herbert Henry Withers, of Walton, near Warrington,
in the countv of Lancashire, a Lieutenant in the
South Lancashire Regiment, hereby give public notice,
that on the twenty-first day of September, one 'thousand
nine hundred and fourteen, I determined to
assume and take and thenceforth 'to use the surname of
Douglas in addition and as a prefix to the surname*
of Withers; and I give notice, that by a deed poll,
dated the twenty-first day of September, one thousand
nine hundred and fourteen, duly executed and attested
and enrolled at .the Central Office of the
Supreme Coiurt, on the 22nd day of September, one
thousand nine hundred and -fourteen, I formally declared
that I should at all times thereafter in all
deeds and writings, and in all dealings and transactions,
and on all occasions whatsoever use the surname
of Douglas in addition and as a prefix to the surname of
Withers, and so as to be at all times thereafter called,
known and described by the name of Herbert Henry
Douglas-Withers exclusively.—Dated the 22nd day of
September, 1914.
